CYANOTIC EPISODE IN A CHILD

Cyanotic episodes spells. Cyanotic spells occur in children with cyanotic congenital heart disease, in particular tetralogy of Fallot and pulmonary atresia. They usually occur early in the morning, or in the context of stress or dehydration ie. periods of increased oxygen demand/ultilisation.

Diagnosis

Occur in cyanotic congenital heart (esp. Fallots tetralogy & pulmonary atresia),

Usually occurs early in the morning or at times of increased oxygen demand (dehydration, feeding, 

crying).

Usually self-limiting.

Assess for:

1. Cyanosis/ pallor,

2. Distress,

3. Dehydration,

4. Lethargy,

5. Evidence of structural heart disease,

6. Absence of previously documented heart murmur.

Management

1. High flow oxygen,

2. Continuous monitoring, 

3. ECG,

4. Correct any arrhythmians, 

5. Liaise early with medical officer.
